Player Parent Code of Conduct (traveling soccer U9-U19)
Each player and parents of U9-U19 traveling soccer teams must sign the Player Parent Code of Conduct (Little Blasters and Intermediate Little Blasters are not included). Click here for the form. This form will be collected by the team manager.

Uniform kit $150-200 (final price TBD). All players are required to purchase the full uniform package. MapleBrook Soccer Association is beginning the 1st year of a 2 year uniform cycle (Summer 2012 – Fall 2013 seasons). The uniform package will include: light and dark jersey, 1 pr. of shorts, 1 pr. of socks, a warm up.

PACT (Parents and Coaches Together)
Consistent with our Mission Statement, MapleBrook's Board of Directors and staff are committed to programs that focus on providing a positive soccer environment for all players, parents, coaches, team managers, officials, and fans.
Motivated by this commitment, MapleBrook is partnering with the Minnesota Youth Soccer Association (MYSA) to provide a research-based, educational program called Parents and Coaches Together (PACT). Co-developed by the University of Notre Dame and MYSA, PACT is designed to help coaches and parents create a positive environment for everyone involved that builds character and thus creates "champions" on and off the field. The value of PACT goes beyond what any individual family may or may not feel they have gained; the real value is what it brings to the club as a whole, the cohesiveness, the commonality of purpose and focus, and the means to make good things happen to make us a better club.
Because of the importance of this initiative, MapleBrook will continue to ask one parent of each new player to the club to attend a PACT seminar. MapleBrook will begin to offer seminars at a variety of times and locations. Once certified, player passes will be issued for participation in the 2011 season. Watch for more information on seminar dates and times on our website and through email communication.
Important Information.
Every new player to the club will be required to show a valid birth certificate, passport, valid driver's license or valid driver's permit to validate the player's age. Also, our club has a volunteerism policy in place: Every player will be require to make a deposit of $100/player up to $200 maximum per family, and 4 hrs. of volunteers hours/player up with a maximum of 8 hrs. The check deposit will be shredded upon completion of volunteerism. In addition, our club has a "No Pay No Play" and "No PACT, No Play" policies.
